All was seemingly well until the last few seconds,

The helicopter then continued in a forward roll such that the helicopter then landed firstly on its roof and then on its port (left) side.

Generally speaking, they're usually not looking at those opportunities unless there's some economic pressure to do so.

The bolt appears to have come out of the bell crank and the question is why that came out.

The nose on the helicopter hit the ground at an angle slightly less than vertical,

It should have remained joined but was apparently not joined and was broken away,

A direction was given to remove the fore-aft bell crank ... and this was done,

There's no proper record of the removal of the bell crank and its reinstallation,

Were you told at any stage in the course of the ... maintenance work that there was any problem with the bell crank or that the bell crank was removed?
